Forms to be completed by participants of C-15-08

Participants of C-15-08 are asked to complete the following forms which can be downloaded from this Web site:

  • Quartermaster Survey
  • Personal Resource Questionnaire
  • Photo Release Form
  • Twenty Questions (for participants to complete and bring with them on the course - for your eyes only!)
Please complete the above forms by August 1.  Any questions, contact Lori Metivier at lmetivier@woh.rr.com.
