Then....

download the Quartermaster Survey and Personal Resource Questionnaire forms.  Fill each of these out, saving them under a new file like "myname_QMSurvey.doc".  You can upload these into to the "Participant Drop Box"  or email them to the scribe (lori.metivier@wright.edu).  REQUIRED forms due by August 1, 2008.

PLAN B - print it, fill it out, and fax it to the attention of Lori Metivier at 937-258-5544 or mail it to Lori Metivier, Wood Badge Troop Scribe, 152 N. Diamond Mill Rd., New Lebanon, OH  45345.
